Our charges are the same for all patients, regardless of whether or not you are insured. VMH does contract with insurance carriers, and those contracts may discount the amount the carrier will actually pay VMH for the services rendered to you. Patients are responsible for all remaining account balances not paid by their insurance plans. This amount will not include any unpaid negotiated discount amount between VMH and your plan.

If you are insured by a government program such as Medicare or Medicaid, your self-pay responsibility is determined by that government program. If you are insured by a commercial carrier, your self-pay responsibility will be determined by your carrier, based on the benefits of your plan.
Please refer to your health plan’s “Explanation of Benefits” summary to view how benefits are paid and how self-pay responsibility is determined. If you have questions about coverage or how your plan calculates your benefits, check your health plan book or call the customer service phone number found on your insurance card.
Additionally, VMH has a unique arrangement in that a majority of surgical procedures are performed by Gundersen Lutheran surgeons. Vernon Memorial Healthcare bills for the surgery and facility charge, while Gundersen Lutheran bills for the professional component of the surgery. VMH and Gundersen Health System are separate entities and, in some cases, do not have the same contractual arrangements with insurance companies.
It is always best to contact your insurance company prior to any medical procedure. VMH accepts most major insurance plans, including:
*Payor plan coverage varies. Not all payor plans from an insurance carrier (Medicaid, Commercial, Medicare) are accepted or in-network. Please check with your insurance carrier to find an in-network provider.

We strive to treat our patients with compassion in regard to payment of their bills and provide various levels of financial assistance, available to those unable to pay their full obligation as determined by our qualification criteria. The amount of aid is determined by a person’s ability to contribute to the cost of his or her care. No one will be denied access to services due to the inability to pay. There is a discounted/sliding fee schedule available based on family size and income.
We offer a Financial Assistance Program (FAP) to those unable to pay for their healthcare services, regardless if you are insured, underinsured or uninsured. For more information on the VMH Financial Assistance Program, please contact us at 608-637-2101 or at paymybill@vmh.org.
Application forms are available below, or you may pick one up from any registration desk in our hospital or network of rural clinics.
