IMPORTANT HEALTH COVERAGE TAX DOCUMENTS: HOW TO REQUEST IRS FORM 1095-C

As permitted by federal law, Auxiant/VMH is no longer automatically mailing federal Forms 1095-C.   If you wish to receive a copy of your Form 1095-C, you may request one by contacting Human Resources as follows:

  • By Facsimile: 608-638-5043
  • By E-mail:  Hresources@vmh.org (due to privacy concerns, the Benefits Office will respond to e-mails via encrypted e-mail only).

Your request must include your first and last name, address, your contact number, and e-mail address (if you want an electronic copy of the Form 1095-C).   You should expect to receive your Form 1095-C within thirty (30) days of your request.
